Impact Lives in the WakeMed Community as a Behavioral Health Counselor, Transitional Care Specialist
As the Behavioral Health Counselor - Transitional Care Specialist (TCS), a licensed clinical social worker, you are responsible for providing transitional care services to patients utilizing emergency and crisis services due to a substance use or a mental health disorder. The Transitional Care team consists of two Transitional Care Specialists and three Peer Support Specialists. Responsibilities include:
Screening all referrals assessing potential patients for appropriateness of transitional care services.
Arranging outpatient services if a different level of care is required.
Facilitating the safe and timely transition from hospital care to community-based services, in collaboration with the Peer Support Specialists.

Bachelor’s Degree in Nursing Required OR Master’s Degree in Social Work or Counseling Required
Master’s Degree in Nursing Preferred
Either of the below is required:
3 Years Clinical - Mental Health
3 Years Clinical - Substance Abuse/Addiction
At least one of the below is required:
Licensed Clinical Mental Health Counselor
Licensed Clinical Social Worker
Licensed Clinical Addictions Specialist and Maintain Vehicle Operator Insurability
Certified Notary Preferred
